The social worker provides psychosocial assessments and develops a corresponding care plan as coordinated with the interdisciplinary team and /or RN Case Manager. What you need for this position: - Bachelors degree in Social Work from a accredited or approved by the Council on Social Work Education. - Current Wyoming license as a certified social worker. - Experience in health care and clinical social work preferred. - Knowledge of the process of death, dying and bereavement required. - Understanding of psychosocial factors operative in terminally ill patients and families required. - Must be able to document and chart per facility policy.
