Home Health Physical Therapist

Description: 

 

Directs physical therapy treatment toward preventing disability, relieving pain, developing, improving and/or restoring muscle function and maintaining maximum performance within the client’s capabilities in accordance with the plan or care/treatment.  Oversees physical therapy assistants to assure appropriate implementation of the plan of care.  Maintaining/and or achieving maximum performance.

 

Position Requirements:

 Graduate of an accredited physical therapy program.One year of physical therapy experience in acute care/home care setting preferred or as required by law.Current valid license as a physical therapist by the state(s) of practice.Current CPR certificationValid driver’s license.

 

Skills: Knowledge and ability and the necessary skill and judgment to perform all procedures as outlined in the scope of practice.Ability to contribute as a team member and ability to work independently.Excellent verbal and written communication skills; strong interpersonal skills.Knowledge of the regulatory requirements at the state, federal, and local level.

 

Essential Functions:

 Evaluates the client’s level of function by applying diagnostic and prognostic muscle, nerve, joint, and functional abilities tests.  Evaluates the home environment for rehabilitation and makes appropriate recommendations to increase the independence and safety of the client.  Supplies physical therapy services in the home as prescribed by a physician.  Administers manual therapeutic exercises; instructs, motivates, and assists clients in non-manual exercises and functional activities. Administers traction, massage and treatment modalities involving the application of physical agents.Evaluates fits and adjusts prosthetic and orthotic devices and recommends any modifications to the physician. Participates in the development, review, and updating of the care plan. Observes records and reports any changes in the client’s condition and response to therapy and makes suggestions for changes in the plan of care to the client’s physician and Case Manager. Develop and prepares clinical records (initial evaluations – including OASIS, care plans, progress notes, discharge summaries) and submits them for the client’s clinical record. Instructs and supervises Home Health Aides or CNAs/STNAs, as applicable, in certain phases of physical therapy services they may to the client.  Instructs the client/family or designated caregiver in the use and care of physical therapy devices and on client’s total physical therapy program. Instructs and supervises Physical Therapy Assistants in certain phases of physical therapy services they may provide to the client.  Completes an on-site functional assessment, review and revision of the plan of care, and assessment of utilization of community resources as required by client.
