While providing direct care to hospice patients in their homes or skilled nursing facilities, the chosen RN would enjoy plenty of growth potential, a friendly staff and an opportunity to interact with patients and their families on an intimately personal level. Primary responsibilities of the Hospice Nurse will include: Collaborating with various healthcare professionals to determine the best overall care for a caseload of terminally ill patients Creating your own daily schedule for patient visits at your discretion Conducting regular visits to residences and/or skilled nursing facilities to meet with members and families directly Working with CNAs, LPNs, social workers and dieticians to provide a wide scope of support for patients and families Coordinating, assessing and implementing patient care Providing accurate and timely documentation of all case management activitiesQualifications: A current, unrestricted RN license in the state of residence is required 2-3 years of clinical experience is required Home care and/or hospice experience Long-term care experience is a plus Excellent communication skills are essential Computer proficiency is a must UnitedHealth Group offers a full range of comprehensive benefits to full-time and part-time employees, including medical, dental and vision, as well as a matching 401k and an employee stock purchase plan. Diversity creates a healthier atmosphere: equal opportunity employer M/F/D/V. UnitedHealth Group is a drug-free workplace.
