Hospice Home Care Nurse

Overview: The Hospice HOMECARE NURSE (RN) provides direct patient care to ensure the effectiveness of the care, treatments and services being provided by the agency, meet the patient's needsResponsibilities: The Homecare Nurse provides skilled nursing services to his/her assigned patients, and communicates progress toward goals and/or any changes needed for the plan of care with the Care Team Manager and/or ADCS, who is responsible for the overall case management of the patient. He/she works under the direct supervision of the Care Team Manager and/or ADCS.Qualifications: A Registered Nurse graduated from an approved school of nursing with a minimum of one year of acute care nursing experience required; previous home care or hospice nursing experience preferred. Licensed as a Registered Nurse in the state(s) in which he/she is required to practice. * Ability to relate to and treat acutely ill/terminally ill patients/families and their particular problems. *Must possess strong clinical assessment, evaluation and patient education skills.
