Hospice Home Health Aide

Job Purpose:

Will perform duties essentially of personal care and palliative care, which would be given to the terminally ill person.  Helps the well family members to carry their share of the responsibilities and encourages the terminally ill member to participate in the life of the family, in so far as able.

Essential Functions:

*Individual will be responsible for delivering care to clients that are on hospice services. 

* Will be responsible to identify and communicate pertinent findings to the RN Case Manager including changes in pain and symptoms.

* Responds to the physical, mental, social and emotional needs of terminally ill patients and their families within the scope of practice.

* Participate in Interdisciplinary Meetings as required.
