Hospice Social Worker - Self directed schedule!

Come to Ovations and discover the rare combination of a rewarding career and the opportunity to make a difference in people's lives. As an Ovations Hospice Social Worker, your primary responsibility will be assisting with the emotional needs and financial concerns for patients who are terminally ill, as well as their families. Serving as a key member of a close team of nurses, volunteers, chaplains and other workers, the key functions include conducting psycho-social assessments and developing care plans, in addition to offering guidance and support to family members. This includes maintaining phone contact, conducting in-home visits and building strong relationships with family members and community programs. Patience and empathy are key for this role.  We’re looking for an MSW or higher with social work experience who is comfortable working autonomously.
