Intake Case Manager

The Community Health Center is a non-profit social service agency in the Akron area providing comprehensive treatment, primary health care, prevention and housing services for addictive and compulsive behaviors and disorders.

We are currently looking for an Intake Case Manager to respond to incoming calls and walk-ins, assist in the referral process, and schedule appointments.

Duties include:Maintain ongoing familiarity with CHC clinical programs to facilitate and coordinate referrals.Appropriately respond to incoming intake and diagnostic phone calls.Insure all necessary paperwork is completed and filed as necessary.Assist in facilitating care of walk-ins and crisis cases.Consult with supervisors, coordinators, directors, clinical director and/or medical doctor as well as utilizing support staff as needed.

Minimum Requirements:MUST be a non-smokerBA in social work or related field preferred or 2 years experience in similar position requiredCDCA or other license or credential preferred
