JOB SUMMARY:
The LPN (Licensed Practical Nurse) Charge Nurse is responsible for the overall management of a nursing unit and for providing nursing care to residents requiring long-term or rehabilitative care. The LPN receives administrative direction from the Director of Nursing (DON), the Assistant Director of Nursing (ADON), Clinical Coordinator, Nursing Supervisor, or designees.
ESSENTIAL DUTIES & RESPONSIBILITIES:
ADMINISTRATIVE/SUPERVISORY
· Exercises administrative and functional supervision over nursing personnel on designated unit.
· Uses independent judgment and exercises ordinary professional or technical judgment in directing less-skilled employees to deliver services in accordance with employer-specific standards.
· Ensures staff documents resident care plans in an accurate and timely manner.
· Responsible for assigning nursing staff based on needs of residents and unit staffing levels to provide appropriate patient care.
· Responsible for assuring appropriate staffing levels through scheduling, assignment of staff to residents, procuring coverage to obtain appropriate staffing levels.
· Directly supervises nursing staff performance, provides ongoing feedback to staff, and completes performance review and competency assessment process based on position description, standards of care, facility and company policies. Recommends performance-based wage increases based on facility and company policy. Recommends promotion of staff when appropriate.
· Identifies staff education needs and recommends appropriate training programs for professional development.
· Addresses staff concerns on an ongoing basis and, where appropriate, initiates disciplinary action based on the facility and company discipline policy.
· Participates in recruitment and staffing processes to ensure appropriate staffing levels on assigned unit(s).
· Provides staff education and training to staff on an ongoing basis. Acts as preceptor for new staff.
· Organizes and monitors human and material resources necessary for meeting the needs of the designated nursing unit.
· Participates in the development of facility unit objectives.
NURSING PRACTICE:
· Practices professional nursing that is consistent with the Nurse Practice Act, Lyric Standards and Protocols, Policies and Procedures and nursing judgment.
· Participates in the interdisciplinary care approach to residents and their families.
· Assists in maintaining safety and cleanliness in resident-care and staff work areas.
· Participates in resident care quality assessment and improvement processes and activities.
· Assesses and documents resident care and progress, including making and documenting professional observations, and reports relevant information using established procedures and protocols.
· Implements resident plans of care and evaluates and documents resident response(s).
· Maintains knowledge of necessary documentation requirements and medication contraindications.
· Initiates emergency support measures (i.e., CPR) and protects residents and staff from injury.
· Provides resident/caregiver education as necessary.
· Maintains confidentiality and resident rights, regarding all patient/resident/personnel information.
· Maintains knowledge of necessary documentation requirements.
· Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors infusion devices, drain devices, etc.
· Works rotating shifts, holidays and weekends and schedules.
· Maintains resident care supplies, equipment and environment.
· Conducts self in professional manner in compliance with unit and facility policies.
· Other duties as assigned.
MINIMUM EDUCATION, SKILLS & EXPERIENCE:
· High school diploma.
· Graduation from a basic educational program in practical (vocational) nursing.
· Current license to practice nursing profession in state where employed.
· A minimum of one (1) year nursing experience in a long-term, sub-acute or acute care setting preferred.
· Ability to work flexible work hours to support state and federal resident care requirements.
· Previous charge nurse or supervisory experience preferred.
POSITION INTERFACES WITH:
· All staff, including Nursing Supervisor, Director of Nursing, Assistant Director of Nursing.
· Residents, residents’ families/visitors.
PHYSICAL DEMANDS:
Conducts work in a healthcare setting. Physical working conditions may involve the following:
· Lifting over 25 pounds.
· Extended periods of sitting.
· Extended periods of walking.
· Extended periods of standing.
· Extended periods of reaching.
· Extended periods of bending.
· Extended periods of reading fine print.
NON-PHYSICAL DEMANDS:
Routine aspects of this position may require the following non-physical demands:
· Analytical and logical reasoning.
· Problem solving and time management.
· Organizational and researching skills.
POTENTIAL HEALTH RISKS/EXPOSURE:
Routine aspects of this position may place the incumbent in situations where they may be susceptible to potential health risks from the following:
· Bloodborne pathogens.

· Body fluids.
