This position is in the Penn Center Rehabilitation and Care Center. In cooperation with the Nursing Supervisor, the charge nurse ensures that a consistently high quality of care is delivered throughout our 124-bed long term care facility. This position is in the Penn Center Rehabilitation and Care Center. In cooperation with the Nursing Supervisor, the charge nurse ensures that a consistently high quality of care is delivered throughout our 124-bed long term care facility. The LPN collaborates with the nursing supervisor daily, to exchange information regarding resident care, resident condition and all matters affecting resident well-being. He/She verifies unit staffing, makes rounds, evaluates resident condition, ensures that CNA staff are making the required rounds, and insures that all residents’ call lights are answered as promptly as possible. Furthermore, the LPN Charge Nurse will insure that the individual nursing care plan is followed, reinforce the training and orientation of nursing personnel and admit, transfer and discharge residents according to facility policy. He/She will contact the attending physician for initial orders on each admission unless these orders accompany the resident in writing and are currently dated and signed by the physician and oversee the residents’ medications. The nurse practices in the role of primary and/or associate nurse within the professional practice model and demonstrates core values with reflect the hospitals mission. The nurse assumes responsibility for the plan of care and utilizes the Nursing process in the delivery of care. The nurse facilitates coordination of care across the continuum. The staff nurse demonstrates safe practice within the domains of clinical judgment and decision making, nurse-patient/family relationship, clinical leadership and clinical scholarship. The nurse maintains a safe level of knowledge and skill with technical procedures and interpersonal communication and also demonstrates the knowledge and skill necessary to provide the expected standards of care. Successful candidate will be a licensed practical nurse in the Commonwealth of PA who is a graduate from a state-approved school of practical nursing and currently registered with the state. He/She will have at least one year (preferably two) experience in long-term care nursing. We are looking for people who want to become experts in their field.
