LPN - Community Nursing - Homeless Outreach and Advocacy

Community Healthlink's Housing First program utilizes the Housing First Model to work with homeless persons transitioning to housing.  The program employs a multi-disciplinary team of providers to advocate for and provide on-going support to individuals who are in the process of securing housing.

PT (20 hrs/wk; benefit eligible) LPN toProvide appropriate medical services in an outreach capacity.Conduct home visits as well as visits to other community sites.Work independently and develop creative solutions to meeting client needs where they are.Work as part of multi-disciplinary to implement and provide care coordination.Implement doctor's orders as directed.

Qualifications

 HSD/GED required. MA LPN license required. Community nursing experience required. Preference will be given to applicants who have previously worked in mental health settings.Preference will also be given to applicants who have previously worked with homeless individuals.
