LPN Health Coach / 110 Position Description:Screens subscriber enrollment data, verifying program eligibility per membership files, and identifies type of Insurance coverage. • Reviews claims data to assist in identifying co-morbid conditions, hospitalizations, prescriptions, and other factors that could impact the educational and support needs of the individual.• Conducts health risk assessments telephonically on individuals that fall under the Disease Management Program. • Obtains verbal consent of the individual for participation in the program.• Identifies problems, sets goals, interventions and determines the individual’s health status based on assessment.• Establishes goals/care plan for each individual regarding educational and health care needs. • Communicates with primary care and/or treating physician to request specific health information and/or to provide an update of an individual’s health care status. • Collaborates an individual’s care plan in conjunction with other health professionals, such as registered nurses, certified dieticians, behavioral health specialists, respiratory or physical therapists. • Transfers cases needing more intensive management due to a higher risk / acuity level to RN health coach.• Transfers cases needing more intensive management when indicated to case management. • Performs follow up assessments and makes contact with an individual in a disease management program as appropriate.• Prepares reports and case summaries as needed.• Completes special assignments and projects instrumental to the function of the department.• Works in conjunction with operational functions regarding clerical staff.• Limited local travel.• Other duties as assigned.LPN License in Wisconsin, Associate degree preferredMedical knowledge and background obtained by completion and licensure as a registered nurse or equivalent behavioral or medical health care professional is required. A minimum of three years active clinical experience in a hospital environment is required. Experience teaching in a clinical setting to patients is desired, as is experience in collaborating with all members of the health care team. Previous knowledge of health insurance or managed care is desired. Excellent oral and written communication, written report preparation, human relations, and decision-making skills are required. PHYSICAL DEMANDS: Typical of an office setting. May need to move, lift, or carry objects weighing 1 to 5 pounds.WORK ENVIRONMENT: The work environment characteristics are those of a standard office setting involving cubicles and/or offices. SCHEDULE: Flexibility of hours is essential to accommodate business/client needs.
