Continuous Care Licensed Vocational Nurse (Days and Nights) The Continuous Care Licensed Vocational Nurse(LVN) or Licensed Practical Nurse(LPN) position is responsible for providing continuous nursing care to terminally ill patients in their home, hospital or long-term care facility. Carries out physician orders and the Plan of Care as directed by the Patient Care Manager or RN Case Manager. Requirements Qualifications Current LVN or LPN license in applicable state. Minimum of 1 year recent experience in hospice, medical, surgical, oncology or home care is required. Demonstrates good verbal and written communication, and organization skills.
