Responsibilities:  1. Coordinate patient care and assure all visits are completed. Delivers direct patient care as necessary. 2. Responds promptly to any patient/family issues and takes appropriate action. 3. Coordinates IDG and serves as member of IDG. 4. Coordinates on-call schedule to assure appropriate coverage and takes call. 5. Assures that staff follow documentation guidelines including accuracy and timeliness and that charts are properly maintained 6. Assures that staff is familiar with policies, procedures and guidelines and are kept informed of changes. Implements team building and staff support activities. 7. Member of leadership team and participates in providing orientation, education, leadership, coaching, accountability and mentoring to program staff. 8. Assures that patients are certified and re-certified appropriately. 9. Ensures compliance with all state and federal regulations 10. Participates in Continuous Quality Improvement activities and assures CQI reports are submitted in accordance with company policies. 11. Monitors patient care costs and staffing model to assure compliance with budget. 12. Responds promptly to regulatory surveys. 13. Assures that all outside services are properly authorized and bills are accurate. 14. Has a duty to detect, when reasonable, and report any potential misconduct or violation of VistaCare Standards of Conduct or federal or state laws and regulations. 15. Has a responsibility to participate in staff training and education programs and to ensure appropriate training of supervised staff.  Qualifications: 1. Meet all state regulations for Patient Care Manager. 2. Two years of nursing experience; prior hospice experience preferred but not required.3. Two years of clinical management experience, or recognized leadership ability. 4. Knowledge of business management and governmental regulations. 5. Bachelors or Master's degree in nursing high desirable. 6. Registered Nurse with current state license. 7. Highly organized, able to handle multiple and shifting assignments and present a professional image. 8. Demonstrated superior oral, written and interpersonal communication skills. 9. Strong team building skills. 10. Demonstrated computer skills including Microsoft Office, Internet and e-mail. 11. Reliable transportation, current driver's license, and current auto insurance.
