License Practical Nurse Positions

3:00PM – 11:00PM Shift

Job Responsibilities:

 

Consistently documents all ongoing clinical data on assigned residents as per MTNRC standards. Promptly and consistently reports findings of data collection to the registered nurse assigned to the resident and other appropriate health care personnel. Reports changes in resident’s condition or inappropriate resident response to nursing interventions to the registered nurse assigned to the resident and other appropriate health care personnel without delay. Demonstrates knowledge of basic laboratory values by notifying the registered nurse assigned to resident and other appropriate health care personnel of any abnormalities. Provides care to residents as delineated within LPN scope of practice, the New York State Practice Act and the policies and procedures of MTNRC. Accurately documents the resident’s response to nursing intervention in accordance with the resident care plan. Strictly maintains resident privacy and confidentiality at all times according to established policies. Demonstrates skills to perform treatments and procedures necessary to deliver excellent and direct resident care. Responds as required in emergency situations. Seeks appropriate resources on the unit. Utilizes appropriate lines of authority and channels of communication. Refers problems that are beyond the scope of individual authority, ability and legal practice immediately to RN or supervisor. Reports hazardous situations without delay. Participates actively in unit based PI Program(s). Provides resident and family education as required or directed by the registered nurse. Collaborates with the registered nurse to update and revise the nursing care plan on assigned resident; actively participates in identifying approaches to meet the continuing care needs of the resident. Addresses the ages and needs of residents and demonstrates knowledge of clinical interventions. All other duties as assigned.

 

 

Job Qualifications:

 

Valid NYS LPN License / Certification. Must have strong organizational skills & the ability to multitask.
