Licensed Practical Nurse ( LPN ) - Health Care Coordinator

LPN Health Care Coordinator opportunity at our Sterling House of Marrimac assisted living residence located in Jacksonville, FL.Day One Benefits!As a Health Ccare Coordinator, we’ll rely on you to provide leadership in the day-to-day operations of the residence support services and directly supervisor the health care given within the residence.  You will manages the health, functional and psycho-social status of residents, initiate individualized service plans and on-going assessments, perform on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment and conduct preadmission screening of prospective new residents.

 

As a member of the residence leadership team, you will provide training, supervision and monitoring of staff in the administration of medications, supervise the maintenance of resident charts, review documentation performed by care giving staff,  and meet with family members on a regular basis regarding their loved ones care.

 

This position requires 24/7 call and may share manager on duty responsibilities.

Requires:  LPN state nursing license and a minimum of 2 years experience working with elderly and memory imparied, working knowledge of personal computing and word process software preferably in a Microsoft Windows environment is required. Supervisory experience and experience working with the memory impaired is preferred.
