Licensed Practical Nurse - home health

Overview: The VISIT NURSE (LICENSED PRACTICAL/VOCATIONAL NURSE) is responsible for providing direct nursing care as prescribed by the patient's plan of care, recording accurate clinical notes, and coordinating assigned patient care activities/services to provide the highest quality service to acutely and/or chronically ill patients in their home environment.Responsibilities: He/she works under the direct supervision of the Care Team Manager, Primary Nurse Clinician or ADCS/DCS. Nursing activities are limited to these in compliance with the State LP/VN Practice Act, any applicable licensure/certification requirements and agency policies and procedures.Qualifications: * Graduation from an approved school of practical/vocational nursing. * Licensed as a LPN in the state(s) in which he/she is required to practice. * A minimum of one year of acute care experience required; previous home care experience preferred. * Ability to relate to and treat acutely ill/chronically ill patients/families and their particular problems.
