The Acute Care program serves clients over 18 years of age with a range of skilled nursing or rehabilitation therapy needs. This program is the primary point of entry for most VNSNY patients. Specialized services are available for individuals with illnesses such as asthma, diabetes, congestive heart failure and cardiopulmonary disease.

As a Licensed Social Worker, you will be responsible for providing social work services to patients and families in collaboration with the health care team consistent with VNS Home Care policies. You will work under general supervision.
This is a salaried Full Time position with benefits.

Current registration to practice as a Licensed social worker in New York State.
Valid driver's license and vehicle.
Master's degree in Social Work.
Minimum of one year social work experience in a health care setting.
Bilingual-Russian Required.
