Licensed Social Worker, Staten Island

VNS CHOICE Community Care is a managed long-term care program for older adults who wish to remain in their own homes for as long as possible, but require ongoing assistance with day-to-day activities due to chronic illnesses and functional decline.

Responsible for providing social work services to members and families in collaboration with an interdisciplinary health care team consistent with VNS CHOICE Community Care policies.
Assesses members, family psychosocial status and social work needs utilizing professional knowledge, skills of
observations and interviewing skills.
Assesses a member’s living condition and situation, cultural influences, and support systems to identify member’s strengths and deficits.
Establishes the social work component of the member/family plan of care based on goals mutually acceptable to the member, family and significant others.
Participates in the development of a care plan and revises goals and approaches to member, family care in coordination with interdisciplinary team members.
Provides initial psychosocial assessments of new members enrolled in the program, and reassesses identified members annually.


Licensure: Current registration to practice as a Licensed Social Worker in New York State required.
Education: Master's Degree in Social Work after successfully completing a prescribed course of study at a graduate school of Social Work accredited by the Council on Social Work Education and the Education Dept. and who is certified or licensed by the Education Dept to practice Social Work in New York State, required.
Experience: Minimum of one year of post-master’s social work in a health care setting preferred.
