MDS Coordinator

SunBridge Care and Rehabilitation for North Reading offers skilled nursing and rehabilitation therapy services to adults of all ages and acuity levels. The MDS Coordinator manages the development, completion and transmission of resident assessment in accordance with the regulatory requirements. The position reviews and manages reimbursement in compliance with SunBridge policies and procedures. Responsible to case manage residents covered by either Medicare or Managed Care insurance programs.



Requirements
We are seeking a caring professional to round out our team.  You must have: An Active Massachusetts RN  or LPN license Long-Term Care Experience Experience with MDS/RAI  process Experience with Care-planning Strong understanding of OBRA regulations and PPS management RN preferred
