Consultation on mandated reporting, eg., child abuse, elder abuse, domestic violence. Consultation & assistance with discharge planning and outpatient member needs, e.g., hospice, or conservatorships. Family conferences & advance care planning. Referrals to community resources. Teen support group & counseling at Antioch Clinic.
