Mental Health Therapist
WORK SCHEDULE:
The normal work week for this position is 37.5 hours each week, Monday through Friday, work week hours may vary.
DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:Maintain caseload of MH and/or DA consumers assigned by supervisor, in accordance with State and County regulations. Complete all documentation within one week of its reporting period. Documentation would include but not be limited to: ISP’s, ISP Reviews, ISP Addendums, consent forms, psychosocial evaluations, PCP letters, release of information authorizations. Secure dated consumer signatures, as appropriate. Conduct individual therapy sessions for consumers as required. Document therapy progress notes during session, reflecting treatment goals; obtain consumer signature, as appropriate. Facilitate group therapy sessions as scheduled. Prepare for group sessions with appropriate research and utilization of group materials. Document group notes for each consumer, reflecting treatment goals. Perform all necessary functions in Qualifacts in order to reflect timely, accurate consumer and staff data. Rotate staffing with psychiatrists. This includes initializing doctor’s orders, copying prescriptions given to consumers, verbally notifying CRR staff of any changes in consumers’ medications via telephone and providing hard copy of doctor’s orders, making necessary changes in prescription binder and assisting physician in other requests to facilitate continuity of care. Complete referrals as needed for Case Management, Resource Coordination, Psychiatric Rehabilitation, Social Rehabilitation and submit to appropriate department. Complete appropriate initial authorizations for managed care companies and request/obtain continued stay reviews as needed within 24 hours. Submit appropriate documentation of authorizations to clerical staff upon receipt of same. Complete documentation as necessary for State, County and managed care audits. This includes appropriate correction of any errors and completion of paperwork within one week of its reporting period. Attend team staffings with psychiatrist and Outpatient staff. Conduct intake interviews as needed to facilitate intake process with BSU, as schedule permits.
REQUIRED MINIMUM TRAINING:
LCSW Medicare Credentialed. Knowledge of individual and group techniques. Knowledge of MISA treatment.
CASEMANAGERS
PRIMARY PURPOSE OF POSITION:Provides case management services to high-risk persons to promote recovery and
reduce the frequency and duration of psychiatric hospitalization; serves as key member of the treatment/service team; meets performance standards.
WORK SCHEDULE: The normal work week for this position is 37.5 hours each week. This schedule, however, may be adjusted to meet the needs of the consumer.
DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
§ Coordinates and facilitates the development of the Individualized Service Plan,
§ Provides input into plan and collaborates with provider-team to ensure that goals are mutually supportive, meet consumer needs and are consistent with the agency mission and professional best practices.
§ Educate consumer and others, such as family members or significant others when necessary, to promote achievement of service objectives and initiate early interventions.
§ Monitors the consumer and assesses emergent needs;
§ establishes linkages and convenes resources as needed.
§ Advocates for and empowers consumer as appropriate.
§ Provides 24-hour crisis services as needed.
§ Represents agency and consumer to other agencies, professionals and the general public.
§ Establishes and maintains effective working relationships with treatment/service team, other resources, consumers and their significant others.
§ Effectively communicates both orally and in writing.
§ Reviews charts for compliance with state regulations and prepares for formal record audits annually by the County, the State, and insurance providers.
§ Uses proprietary software document services in accordance with policies, procedures and requirements.
§ Meets performance standards.
§ Documents involvement in hospitalization decisions;
§ Documents involvement with consumer during inpatient stays and arranges for aftercare appointments within 7 days of discharge from inpatient or RTF;
§ Ensures that consumers who are on medication are seen for medication management sessions at least one time per quarter, and that less than 25% of assigned consumers require crisis services;
§ Provides 16 or more units of service to 70% or more of new consumers within 30 days of authorization, while delivering less than 8 units of service per month to no more than 20% of caseload;
§ Reduces inpatient readmissions within 30 days of inpatient discharge;
§ Face to face contact with assigned consumers must be made at least twice a month.
§ Generates 1300 or more hours of service per year.
REQUIRED MINIMUM TRAINING:A Bachelor’s degree in social work, psychology, or related field and three years of professional experience providing human sciences.or an RN license with 2 years experience in related field. AND Current valid PA Class I drivers license, Act 33 and 34 clearances
