MR Day Specialist I

JOB DESCRIPTION:

Provide input and assistance in day-to-day training and supervision of individuals in the program.  Directly assist individuals in all group activities and community outings.  Assist in developing IPP goals and prepare materials to meet these goals.  Monitor and document individual’s progress.  Attend meetings, as requested by the supervisor.

 SPECIFIC DUTIES:

1.  Provide assistance and supervision of assigned individuals during group and individualized activities.

2.  Assist in development and review of annual assessment for assigned individuals.

3.  Assist in development and review of IPP’s for assigned individuals.

4.  Attend and participate in IPP meetings for assigned individuals, as requested by supervisor.

5.  Prepare materials necessary to further IPP objectives.

6.  Monitor individuals progress and document in the form of data collection, monthly progress notes, and production cards, if indicated.

7.  Assist in coordination of training and activity planning.

8.  Drive van for programmatic reasons (during program hours) on an as-needed basis.

9.  Monitor individuals as they are waiting to go home after the program ends each day, as needed.

10.  Attend staff meetings and trainings scheduled.

11.  Must receive 24 training hours.

12.  Other related duties, as assigned.

 

QUALIFICATIONS:

High School degree and a minimum of one year of experience working directly with individuals with disabilities, or Associates or 60 credits with less than one year of experience working with individuals with disabilities.  Valid driver’s license and ability to drive a 15 passenger van preferred.  Physical examination certifying no presence of communicable disease, including a TB test no more than 12 months prior to employment and every two years thereafter.
