MRDD Medicaid Service Coordinator
Making a difference since 1946, Cardinal McCloskey Services (CMS), is a well respected not for profit human service agency providing abused and neglected children, at risk families and developmentally disabled adults with the hope for a brighter future by providing countless hours of service each year to more than 3,100 and 2,000 families. CMS provides care daily to more than 1,500 children and their families and nearly 100 adults and teens with mental retardation, helping to enhance the quality of their lives and support the sanctity of family.
CMS is recruiting for a Service Coordinator for our MRDD program. The purpose of the position is to develop, implement, and maintain individual service plans for individuals with disabilities in lower Westchester, and the northern Bronx.
Position Responsibilities:
Providing high quality, person centered servicesTo provide good communications with service providers and families/advocates.Completing all necessary documentation- Monthly Note, ISP, SCOR’s and LOC’s within required timeframes.To meet all Medicaid billing requirements through documentation.To complete semi-annual Service Coordination Observation Reports, reviewing the report and if any problems are noted, sending copies of the report to the responsible agency and ensure that appropriate action is taken. To develop the ISP’s and complete the Service Coordination Agreements Form properly with signatures, dates and attachmentsTo complete Level of Care Forms yearly or as needed.To complete all necessary forms to ensure SSI/Medicaid eligibilityTo attend programmatic and administrative supervisionTo confirm that the service coordination activities identified in the Service Coordination Agreement are appropriate for obtaining or maintaining the person’s goals.Review and follow up on any evidence that the personal or civil rights have been denied or abridgedTo advocate for the rights of the consumer.To take all reasonable steps to ensure that all health, welfare and fire safety needs are met.To take reasonable steps in order to comply with Part 624, 633, and 635 regulations
The successful candidate with have a bachelor's degree in human services or related concentration with at least one year previous experience with the MRDD population. Candidate must have a valid driver's license (use of own car is required). Excellent computer and writing skill are required. Successful candidate may have the option of working from home.
