MSW CASE PLANNER

Northern Manhattan Perinatal Parnership (NMPP) a maternal and child health agency located in Central Harlem seeks an MSW case planner for its Administration for Children Services (ACS) Program.
The MSW Case Planner provides services to high risk families for core services and to ensure the well being and safety of children in Central Harlem. Provide individual, joint, and family therapy and counseling. Should have group experience, prepare progress notes and have strong writing skills. Should be familiar with the CNNX and PROMIS reporting systems. Responsible for advocacy, referral and collaboration of services for clients to daycare, housing, housing subsidies, perinatal and postnatal care, mental health services etc. to improve the families quality of life. Assist the families to learn new behavior, techniques, and concepts that will strengthen the family communications, home management skills, budgeting skills, and parenting skills.
