QUALIFICATIONS: * 2-5 yrs experience as a Social Worker. Previous hospice or home care experience helpful. * Master's or Bachelor's degree in Social Work.  * Experience with local community resources. * Experience working with the elderly. * Excellent communication, time management and customer service skills. * Reliable transportation, current driver's license and current auto insurance.
