OVERVIEW OF THE POSITION:
Responsible for the coordination and management of programs and staff for Cardio-Pulmonary Diagnostics, Cardiac Rehabilitation and Pulmonary Rehabilitation.
ESSENTIAL FUNCTIONS:
□ Consistently exhibits behavior and communication skills that demonstrate HealthCare Partners’ (HCP) commitment to superior customer service, including quality, care and concern with each and every internal and external customer.
□ Performs needs assessments, program development, maintenance and evaluation, referral maintenance, and program marketing and visibility to network, patients, staff and providers.
□ Coordinates department operations including staffing, scheduling, hiring, discipline, counseling, termination, orientation, training and performance appraisals.
□ Monitors and reports to management on budgets, monthly/yearly statistics, program utilization, referrals and staffing.
□ Interacts with other facilities and departmental management and staff to ensure that Cardio-Pulmonary services required in their areas are delivered in the most efficient manner.
□ Works with Health Enhancement and Care Management to ensure proper referral and follow-up.
□ Develops and maintains departmental policies and procedures, business plans, and accreditation standards.
□ Functions as an RN as needed in all departmental clinical areas.
□ Uses, protects, and discloses HCP patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
□ Performs additional duties as assigned.
EDUCATION:
° Bachelor’s degree from a four-year college and/or a professional certification requiring formal education beyond a two-year college.
° California RN license.
° BCLS and ACLS certification.
° Degree in Nursing preferred.
EXPERIENCE:
° Over 5 years and up to and including 7 years of experience.
° Experience working in and managing cardiac and pulmonary rehabilitation.
° Experience in diagnostic cardio-pulmonary testing (includes treadmill testing, Holter monitor interpretation, pacemaker follow-up and pulmonary function testing).
° Experience in designing, implementing and evaluating patient education programs.
KNOWLEDGE, SKILLS, ABILITIES:
° Computer literate.
° Extensive clinical knowledge in cardiac disease and its treatment.
° Ability to interpret EKGs.
° Effective verbal and written communication skills.
° Ability to teach staff and patients.
° Bilingual in English and Spanish preferred.
