Maternal Child Health RN

Maternal/Child Health RN Copy Facility: Caritas Home Care / Norwood Office, Norwood MA Department: Home Care - Full-time - 8:00 am - 4:30 pm, and every 4th weekend. - Day shift - Licensure Required - 1-2 years acute care experience is required - Basic computer skills required - Customer service skills are required - Home Care experience is preferred Provides nursing care to patients and families in their homes, which supports the total person. This is accomplished within an interdisciplinary plan of care. Participates in orienting and supervising Home Health Aides. Works collaboratively with managers, supervisors, Home Health Aide's, Social Worker, P.T., O.T., S.L.P., and office support staff to coordinate patient care utilizing community resources as needed. Participates in staff education. Provides consultation regarding assessment, planning and implementation of care. Coordinates activities with other community, agency and/or department staff and carries out special activities on assignment.
