Medical Reconciliation and Surgery Department Pharmacists Needed!

Medical Reconciliation Pharmacist:

This is a unique position involving pharmacists directly involved in patient care at admission, throughout hospitalization and at discharge. The Medication Reconciliation Pharmacist is part of the admission team, responsible for obtaining patient medication histories, chart review, admission medication reconciliation and admission medication orders. The Medication Reconciliation Pharmacist will work with the Medical staff  in pharmacotherapeutic decision making through hospitalization to discharge medication reconciliation, creating discharge medication lists for patients and providing education to patients and/or caregivers. In addition, drug distribution functions include: Review of medication orders, entering medication orders into pharmacy computer system, dispensing, filling and distribution of medications, floor inspections, control drug distribution, unit dose and extemporaneous dose packaging, IV and chemotherapy admixture, supervision of Pharmacy Technicians. Maintaining drug profiles, counseling patients on appropriate drug use, anticoagulation monitoring, reviewing records to help prevent inappropriate or misuse of medications, provide clinical services, provide medication inservices to ancillary hospital personnel and other duties as assigned by pharmacy administration.


Surgery Department Pharmacist:

Provide normal range of pharmacist functions such as clinical, consultative and dispensing services to a specific group of patients located on an medicine, surgical, pediatric and/or ancillary personnel. Review medication orders for incompatabilities. Accuracy and compliance with applicable state and federal laws and regulations. Prepare relevant articles for Department's publications.
