Position Duties / Details:

 

· The Mental Health RN Case Manager provides a full range of nursing care to patients with complex care needs, assuming responsibility and accountability for nursing actions.

· Completes initial assessments and re-assessments of patient condition and response to treatment and other clinical findings, initiates follow-up as needed.

· Assesses patient’s learning needs, develops and implements a plan to meet needs, and instructs patients and families in self-care and treatment measures.

· Promotes continuity of care by developing an individualized plan of care in collaboration with the patient and health care team, involves community resources as appropriate.

· Administers prescribed therapies using appropriate judgment, initiates referrals to other disciplines and/or services as appropriate, documents in the medical record patient findings, assessments, and care provided, administers medication in accordance with established policy/procedures.

 

Experience/Qualifications Needed:

 

· Minimum 1 year of experience in Mental Health Case Management

· Current, unrestricted license to practice.

· Basic Life Support Certification

· Registered Nurse AD or Diploma; BSN Preferred

 

Hours:

 

Monday through Friday, 8 hour days.
