As part of the Telecare community, The Telecare Recovery Access Center (TRAC), includes three high intensive programs including Westside Stanislaus Homeless Outreach Program (SHOP), the Partnership TRAC and Josie’s TRAC (serving transitional age youth) along with a moderate intensity program called Fast TRAC. These programs provide a continuity of care and a variety of treatment options ranging from high intensity  Full Service Partnership (FSP) programs using the Assertive Community Treatment (ACT) model to low intensity member-supported Wellness TRAC These programs are funded by Stanislaus County Department of Mental Health and the Mental Health Services Act (MHSA). This variety of options supports individuals in all levels of their recovery while offering choice and flexibility to members and their families. All of the programs offer case management and psychiatric services at the core as well as 24/7 crisis intervention services

As a Clinician you will plan, develop and implement supportive services and activities that promote mental and physical health and well-being, and support members in their recovery and wellness.

 

Requires a Master’s degree in Social Work or other health care degree along with (1) year mental health experience or other allied health profession or BA/BS in Social Work, psychology and 3+ years' mental health experience. Licensed or License Eligible is a big plus (LCSW, RN, LMFT). Valid CA driver’s license is necessary. Must complete first aid and CPR training or obtain certification within four (4) weeks of hire. Background in vocational, housing, or dual diagnoses services preferred. Bilingual is a big plus.
