As the newest member of the Telecare family, Yolo Strides is an intensive community support service for 50 adults with mental illness, many of whom would otherwise require extended care in institutional settings.  Services are provided to Clients living in the community (homes, transitional housing, hospitals, etc).

 

In this leadership position, you will utilize your Psychiatric Rehabilitation principals and Recovery Philosophy providing clinical and administrative supervision, coaching and education for assigned team members. You will provide direct customer, community support, crisis intervention and case management to Clients. In addition, you will coordinate service needs with county agencies and will determine the appropriate level of service for new Clients.

 

To be a success, you must be licensed (LCSW/LMFT/Psy.D/Ph.D) or licensed eligible. 2 years’ of direct clinical service is required. Outpatient clinical or community mental health service is preferred. Experience providing intensive case management, intensive community supports using psychiatric rehabilitation concepts is highly desirable. A current valid CA drivers license, and CPR certification are required. Applicant must be fingerprinted and receive clearance from Criminal Justice Department. Must be willing to transport Clients in own car.
