Mental Health Worker/Case Manager/Service Coordinator I

The Mental Health Worker assists consumers to develop the necessary living skills and gain access to local community medical, social, and vocational resources to resolve issues of healthcare, housing and employment. The Mental Health Worker provides services in a variety of settings (home, school, field, hospital) by engaging consumers in a culturally appropriate setting, using available supports within the community such as family members/significant others. Some individual may have chronic and severe mental illness.
Coordinates with school staff, clinic staff MD, DMH and community resources in the treatment planning provides linkage to services in the community or otherwise as needed to support consumer’s treatment Completes all documentation of client care according to Clinics guidelines

Qualifications Associate, Bachelor or Master’s degree or relevant experience working in behavioral healthcare, day treatment, outpatient setting or community mental health setting.

Must be able to perform independently under general supervision and demonstrate ability to work as part of a multidisciplinary team
Valid CA driver license and insurability on the Clinics' auto-liability insurance policy
