Nurse Care Coordinator

Colorado Access is a nonprofit health plan serving the medical and behavioral needs of the medically underserved in Colorado. We offer an excellent benefit and compensation package, including medical, dental & vision, 401(k), and competitive PTO.
We are currently seeking a Nurse Care Coordinator who is interested in expanding skills while working for a company that gives back to the community. The successful candidate will be responsible for working in collaboration with the Clinical Support Services team to identify members through utilization or disease specific indicators who require care coordination. Care coordination responsibility encompasses working directly with members, families and providers over time to assist in arranging and managing a complex set of resources required to maintain health and function.
Requirements:Nursing degree. Bachelor’s degree preferred or may substitute combination of relevant education or experience. Continuing education in care coordination and other relevant area desired.Knowledge of managed care, case management and utilization management;Must have excellent written and verbal communication skills and a working knowledge of fiscal responsibility;Must have strong organization (time management) skills, strong interpersonal skills and the ability to handle multiple priorities; Working knowledge of word processing and spreadsheet programs, preferably in Microsoft Office, and case management software for data management;Ability to use the complaint, grievance and appeal process and procedures to facilitate quality outcomes and/or resolutions for the benefit of the members, providers and Colorado Access;Valid Colorado Driver’s License and current unrestricted Colorado healthcare professional license.
Experience:3 years of clinical, case management and healthcare experience required;Acute care experience required;Experience with utilization review and quality improvement processes required;Experience working with Medicare and Medicaid populations and related social/economic issues preferred;Experience with ICD-9, CPT4 and DRG coding preferred.
Bi-Lingual a plus.
