Nurse Case Manager Needed in LTACH Arena -  North Valley Hospital

We are searching for a qualified Nurse Case Manager for mostly our 54 bed LTACH.  This position has accountability for the comprehensive coordination of care for all program specific patients at North Valley  Hospital.  The Case Manager understands and integrates into the delivery of patient care, the needs of adolescent, adult and geriatric patients in regard to their treatment needs and their continued growth and development.  Serves as primary person to clarify and report results of treatment team conferences to patient/families.  Assists in planning, developing, implementing treatment and discharge/follow-up service needs.  Actively participates in coworker support, program development and program accreditation activities.  Also is responsible for insuring that high quality, cost effective financially supported services are provided to patients in a timely manner to meet quality standards and patient satisfaction.  Plus manage the financial aspects of the patient's care for efficiency and positive outcomes and appropriately documents and communicates the results of these services. 

 

Qualifications are as follows:

Education
Clinical License needed, RN required.

Professional Experience
Minimum two years experience in a medical setting with a minimum of one year in brain injury experience and case management; rehab and/or psych experience preferred.

Technical Skills
Observation, listening, verbalization and negotiating skills.  Fluent in the English language, both written and oral. Competent in use of office equipment.
