Nurse Case Manager - Utilization Review

Reputable hospital in lower Manhattan is seeking a Nurse Case Manager. This is M-F day position.

Salary is $75k-$85k. Excellent benefits.

The Nurse Case Manager is responsible for coordinating and integrating high quality, cost effective care delivered to patients across an episode of illness or throughout the entire continuum of health care delivery. You will assess the patient’s clinical status, and the patient’s/family/caregiver’s ability to understand patient’s care needs and make care decisions post-hospitalization, and collaborate with a social worker to evaluate and document cases according to state guidelines. You will develop collaborative relationships with all members of the healthcare team; coordinate activities including social services, respiratory therapy, nutrition, PT, pastoral care, etc.; ensure appropriate documentation of medical records; and maintain a working knowledge of insurance requirements.

RN with current licensure and ASN/BSN or equivalent degree; Master's preferred. Three to Five year's clinical experience in acute care setting; one year case management in an inpatient, HMO, Insurance or other outpatient setting required; one year management experience preferred. Strong leadership, organizational, and interpersonal skills, and basic computer skills - knowledge of MS Word, Access and Excel highly desirable.
