Nurse Liaison - External Case Manager

Come join our Team! SunBridge Care and Rehabilitation for Siler City is a 150-bed center located in Siler City, North Carolina who is seeking an Nurse Liaison with long term care experience.

We offer competitive pay and full time include full benefits package with medical, dental, vision and 401(k).This position is responsible to develop and build a strong network within the geographic assigned territory to facilitate referrals, support the coordination of services among the areas’ providers (i.e. hospitals, physicians, etc.).

You will market the center and its programs. Our ideal candidate will have great marketing, assessment & time management skills. The ideal candidate will have experience as a nurse liaison with marketing/sale experience

Responsibilities include:

• Development & execution of external marketing plan for the center

• Market and promote facility in order to maintain acceptable census and facility profitability.

• Developing & maintaining referral sources, education others about the company, increasing visibility and reputation in the medical community, and identifying market needs and trends

• Conducting comprehensive screen of patients, keeping facility capabilities in mind, and working with appropriate facility staff to facilitate the admission.





Requirements
* Registered Nursing ASN degree required; BSN preferred.

* Current RN license in state of North Carolina.
