Autonomous, field-based position. We are looking for several Nurse Practitioners to provide primary care services for frail and elderly patients in a long-term care setting. Our valued NPs manage their own caseloads as they collaborate with primary care physicians and nursing home staff to maintain the health and well being of our enrollees. Primary responsibilities include: Providing direct care to patients in long-term care facilities Driving from facility to facility on a daily basis Collaborating with primary care physicians and nursing home staff to assess needs and review plans of care Going on rounds with physicians as needed Participating in care conferences with medical staff, members and members' families (in person and over the phone) Communicating on a regular basis with clinical advisors or service managers to review the caseload, discuss transfers and assess risk Providing education/in-servicing to nursing home staff and physicians The chosen candidate must be a nationally certified NP (Geriatric, Adult, Family or Acute Care). Previous experience in a long-term care setting is preferred. New NP graduates with RN experience, ideally in a long-term care setting, will also be considered.
