Primary Care Practitioners:  NP, RN  AND PA

Are you an Nurse Practitioner (NP), Registered Nurse (RN) or Physician Assistant (PA) looking to put your primary care skills to work in a non-traditional setting?  Look no further!  We are Commonwealth Care Alliance (CCA), a nationally recognized innovative healthcare delivery system focused exclusively on the needs of elders and individuals with disabilities of all ages.  CCA brings primary care to the person, whether at home, day program or other setting. 

 

The CCA program is seeking NPs, RNs and PAs for its Senior Care Options (SCO) Program and Special Needs Plans (SNP). The positions in CCA present an opportunity for the NP, RN & PA to develop an enhanced skill set while working in a multidisciplinary team model of care. In this role, you will provide enhanced primary care, care coordination and care management services including needs assessment, care planning, and treatment and you will function independently but in close collaboration with community physicians and geriatric social workers.

 

REQUIREMENTS:  Prior nursing experience is required (3+ years); Ability to speak and understand Spanish - is an added plus or any other language fluently (since we serve such a diverse population); Valid driver’s license and car necessary.  Competitive salary, benefits and AGGRESSIVE SIGN-ON BONUS and many other perks, as well!!
