Advanced Therapy & Rehab (AT&R) is a division of Boulevard Healthcare which provides rehabilitations services within their five skilled nursing facilities located in Illinois and Ohio.  Our large, dedicated rehab facilities contain state of the art equipment unitizing the highly successful Restore Program.  Treating a wide variety of symptoms, our healthcare professionals provide individualized treatments to maximize outcomes and provide compassionate, quality care.  The majority of our patients are with the geriatric population with orthopedic, neurological, general deconditioning and cardiopulmonary Dx.

 

Located in our Evergreen Healthcare Center, Evergreen Park, IL we are looking for clinicians who are willing to make the commitment to continuously improving their skills through mentoring, continuing education, and by being an active team member.

Occupational Therapy Assistants- COTA

Job Summary:

Implements high quality occupational therapy services for patients in accordance with state regulations, principles and practices of occupational therapy and AT&R policies and procedures.  Perform patient screening, evaluation and treatment while documenting progress in a timely manner.  Enhance patient care using appropriate adaptive and exercise equipment in a rehabilitation and skilled nursing facility.  Supervises assistants and aids.  Maintain postive level of interaction with patient, families, rehab team and the facility.

 

Qualifications:

Current and valid state license to practice in Illinois
