Physician Health Partners, a premier health care management provider, is seeking a detailed oriented and collaborative Case Manager Onsite Review, who has expertise in reviewing utilization of medical services, quality of care, risk management and can provide intervention for case management in a timely manner to help with problem prevention and resolution.

 

Requirements 

The right candidate will be able to assess patient’s medical and psychosocial needs to develop and communicate a comprehensive plan of care in collaboration with the health care team, based in timeliness and good utilization of financial resources.

 

The successful candidate will have had success with establishing and maintaining effective work relationships within and outside their organization and will be required to develop working relationship with medical and hospital staff.  Can illustrate solid communication skills that will help reach mutually established goals in a timely manner and ability to interpret medical data to non-licensed staff. 

 

This person will be eager to use various data and reports to continuously improve the process and prevention of problems. In addition, we will be looking for someone who is knowledgeable and innovative in the area of case management.

 

·  Three (3) - five (5) years clinical nursing experience

·  Knowledge of Milliman Care Guidelines.  Previous UR/QM, Case Management discharge planning experience is preferred.

·  Basic knowledge of Microsoft Outlook, Word & Excel.

 

* This position requires travel in and around the Denver Metro area and requires a valid driver’s license.
