Outreach Care Advocate - Psych RN, PhD, LCSW, LMFT or LPC

Our LifeSolutions outreach program is a unique member-centric program that focuses on improving health outcomes for high risk medical individuals with behavioral health needs related to depression, stress, anxiety, substance abuse and other psychosocial issues. We seek a flexible team player with a mental health license to fill an exciting, telephonic Outreach Care Advocate role in our San Francisco office. This position will offer the right candidate a chance to truly make a difference in members’ lives by identifying risks on the front-end and linking medical and behavioral treatment to create a complete care plan for the patient. If you are interested in working with a new and unique behavioral health program, apply today. Focusing on high-risk patients with depression, stress, anxiety, substance abuse and psychosocial issues, your primary responsibilities would include: Calling outpatient members to conduct assessments Providing referrals and resources Coordinating medical, substance abuse and mental health care needs and care between various clinicians, facilities, support groups and programs Communicating with members' family members as needed Developing individual plans of care that focus on patients' mental health needs and stressors as they relate to medical conditions and treatments Coordinating with Medical Case Managers, Disease Managers, etc. Required qualifications include: One of the following (licenses must be current and unrestricted): RN, PhD/PsyD, LCSW, LMFT or LPC Solid mental health experience (including outpatient) Ability to conduct telephonic assessments Good typing skills PC proficiency Master's degree or above (BSN for RNs) Preferred qualifications include: Inpatient experience Case management experience EAP background Experience with chemical dependency/substance abuse Knowledge of community resourcesUnitedHealth Group offers a full range of comprehensive benefits to full-time and part-time employees, including medical, dental and vision, as well as a matching 401k and an employee stock purchase plan.
