Patient Care Manager, Home Care

Hospice of the North Shore (HNS), a community-based not-for-profit hospice and palliative care organgization and Massachuestts' leading provider of end-of-life care, is seeking a Patient Care Manager to join our Home Care Team.

Serving 44 communities north of Boston, HNS has a strong continuum of care, from palliative care consultation to comprehensive bereavement support services.  Services are provided in diverse settings including homes, hospitals, skilled nursing facilities, assisted living facilities, and our freestanding 12-bed hospice inpatient facility.  Visit our website at www.hns.org

As a full-time member of our Clinical Operations management team, you will manage and coordinate services provided to hospice patients and families by an interdisciplinary team of nurses, social workers, and chaplains.  You will be responsible for staffing, orientation, clinical competency assessment, and performance management, and for oversight of home health aide and contracted services.  In addition, you will make recommendations and decisions regarding changes in level of care for patients and monitor the effective use of hospice benefits to ensure cost effective delivery of care.You will perform patient record review to ensure compliance with internal standards and external regulatory requirements. Position requires a Registered Nurse currently licensed in Massachusetts, preferably with a BSN with a minimum of 3 years related clinical experience preferably in hospice, home health, or a long term care setting, and  3 years experience managing clinical staff.  Ability to take periodic administrator on-call coverage required.
