Patient Service Manager - Full Time (Manhattan)

Congregate Care provides care to elderly, disabled and chronically ill individuals who live in a variety of housing settings, such as adult homes, assisted-living facilities, Section 202 and other senior citizen housing, co-op apartment developments, single room occupancy (SRO) facilities, and naturally occurring retirement communities. Our skilled nurses work on-site to deliver and coordinate care. In addition, they are available to provide health information and education to site residents. Our focus on the needs of resident populations provides for an efficient and cost-effective service delivery model.

As a Patient Service Manager, you will be responsible for managing multi-disciplinary health care teams and other teams, as assigned, to ensure that standards for service delivery and team performance are met or exceeded. You will provide key leadership in facilitating and promoting the achievement of the VNSNY mission, guiding principles, philosophy of clinical practice and agency goals and strategic objectives. You will work under general direction of the Regional Administration or Program Director, and under general supervision.

Licensure: License and current registration to practice as a Registered Professional Nurse in New York State required.
Education/Experience: Bachelor’s Degree in Nursing and minimum two years experience in home care required, OR, A combination of education, experience and/or training which includes a minimum of four years experience in home care, plus six credit hours, or the equivalent, of education/training in Public Health and Principles of Management required.
