Home Health Care Management, a nonprofit organization in southeastern Pennsylvania whose objective is to provide operational support to our family of home health care agencies, has an immediate opening for a full time Performance Improvement Director with a Home Health focus to support the organization’s mission to continuously improve the quality and service of care.  Founded in 1987, Home Health Care Management built its reputation by supporting our sister agencies, Berks Visiting Nurse Association, Visiting Nurse Association of Pottstown & Vicinity, Greater Lehigh Valley Visiting Nurse Association and Advantage Home Care, serving Berks, Montgomery, Chester and Lehigh counties.  Headquartered in Wyomissing, Pennsylvania, HHCM and our family of companies are accredited by the Community Health Accreditation Program and State licensed as well as Medicare and Medicaid certified. 

 

The Performance Improvement Director coordinates all PI activities for the Corporation, manages a 6 member clinical department, develops, implements, monitors and reports on the activities of the Performance Improvement Program; uses performance improvement methodology and data analysis to assist and guide Home Health managers and staff in performance improvement efforts; and works collaboratively with all members of the Performance Improvement Department to assure delivery of high quality home health service in accordance with programs and policies; assists in the monitoring of processes and data entry related to computerized documentation, coding and billing practices to ensure timeliness and accuracy; assists in evaluating computer related activities of the sister agencies through quality improvement processes; reviews computer generated reports on a continuous basis, assists with orientation and retraining of staff; and promotes and assists in maintaining an environment that is in compliance with state, federal, local and CHAP regulations. 

 

JOB REQUIREMENTS: 

· Registered Nurse licensed to practice in the State of Pennsylvania

· Bachelor's Degree preferred

· Two to three years of health care quality management experience, Home Care experience preferred.

· Experience with Oasis data management, and Microsoft Office.

· Knowledgeable of home health, state, federal, local and CHAP regulations.

· Excellent organizational skills, problem solving abilities, strong communication skills, and the ability to lead effectively, both orally and in writing.

· Excellent interpersonal skills, a team player, and an effective leader.

 

Must be able to sit for a number of hours, drive to other off-site locations, stand, walk, relate to others, write, speak clearly in person and via telephone, make independent judgments, listen and have good analytical and problem solving abilities.
