Demonstrated knowledge is required in the following areas: IV hydration, pain management, infectious disease, chemotherapy, IV admixture, nutritional support, monthly and quarterly chart reviews, Medi-Cal formulary, Medicare DMERC and restrictions, and third party reimbursement plans. This position assures that all medication orders are consistent with current medical practice, Board of Pharmacy Regulations, F.D.A. and drug manufacture recommendations, current literature and/or other information sources, and drugs are prepared and distributed according to current JCAHO guidelines.Bachelor of Science or Doctor of Pharmacy degree from a recognized pharmacy school. Current California pharmacy license. Residency in Home Infusion or Hospital Pharmacy is desirable. Previous experience as a Home Infusion/Infusion Clinic/Hospital Pharmacist is desirable. Demonstrated leadership and team building skills are desired. IV hydration, pain management, infectious disease, chemotherapy, IV admixture and nutritional support experience is desired. Drug utilization review procedures, therapeutic drug monitoring, and knowledge of quality improvement. Knowledge in the preparation of monthly and quarterly pharmacy reports is required. Computer knowledge is required.
