Under moderate direction, coordinates and assists in managing/participating in pharmacy services which include compounding, IOP activities, dispensing, and clinical monitoring activities. REPORTS TO: Pharmacist-in-Charge/Pharmacy ManagerSUPERVISES: Pharmacy technicians, pharmacy interns, laboratory technicians, delivery personnel. To possess the knowledge, skills, and current competence and proficiency in the appropriate pharmaceutical care services assigned based on the scope of service. To participate in decisions regarding pharmaceutical care services within their assigned scope to include: assessing the patient's general pharmaceutical needs. planning for the provision of pharmaceutical services. participating in the overall plan of care. participating in the overall IOP plan. Receive physician prescription orders and evaluate the appropriateness of these orders for dosage, drug choice, potential drug interactions, route of administration and stability. Ensure that the patient database is complete for height, weight, current and past medical history, clinical laboratory results, and concurrent medications (including over-the-counter or home remedy medications). Assist in the maintenance and upkeep of all necessary paperwork required by state and federal agencies as well as the Joint Commission. Ensure that when necessary, laboratory tests have been ordered and the results are obtained and discussed with the nurse and the physician. Assist the physician, nurse, and patient with clinical and technical support. Assist the pharmacist-in-charge in implementing new procedures. Assist in the training and supervision of pharmacy technicians, pharmacy interns, laboratory technicians and other pharmacy personnel Directly supervise all sterile compounding . Document all communication with physicians, nurses, and patients in the patient chart. Ensure safety to patients and personnel by exercising good pharmacy practice. Participate in the process of providing drug therapy instructions to the patient. Participate in inter-disciplinary patient care conferences. Assist in providing in-service programs to nursing personnel on any aspect of drug therapy. Is expected to perform other tasks as determined by the pharmacist-in-charge/pharmacy manager. Exposure to hazardous chemicals/waste may occur. Since these agents may cause harmful effects, individuals will have medical examinations. Education: Graduation from an accredited School of Pharmacy. Current licensure in state(s) of practice. Current license in all states, which are serviced by the facility or organization. Completed an aseptic technique program/course (e.g., ASHP program). Experience: Minimum 1 to 2 years experience in hospital pharmacy. Good oral and written communication skills. Completed employer approved hazardous chemical handling and waste disposal training program. Home care experience a plus. Good inter-personal skills.
