Position Description:

Working alongside with the physicians and nurses, Hoag Pharmacists provide pharmaceutical care services to our patients as well as other health care providers. Pharmacists utilize his/her clinical skills to assist physicians with processing medication orders and managing medication therapies according to approved guidelines. Specifically, pharmacists are involved with pain management, pharmacokinetic dosing, antithrombotic therapy, nutritional support, IV-to-PO dosing, and Automatic Therapeutic Interchange. Hoag Pharmacists are a valuable and integral part of the medication management system.Performs job functions independently during the night shift to support the above activities.

Qualifications:

• Graduation from a pharmacy program with at least a B.S. degree. • Must possess a valid license to practice pharmacy in the State of California. • Minimum of 1 year hospital pharmacy experience or completion of an ASHP-accredited residency program preferred.
