Occupational Therapist will be a professionals that provides care as needed for our patients in a home setting.

You will oversee the OT needs of patients and work with other professionals to manage patients suffering from CHF/COPD or in need of anticoagulation therapy due to total hip/knee replacement.

Average visits require the OT to assess patient, provide teaching, manage risks, and work with other disciplines to improve patient outcomes.

Requirements
* Licensed in good standing by the State of Texas;
* Experience with geriatric patients a plus; and
* Able to substantiate improvement in patient outcomes.
