Physical Therapist - Pool C

Requirements: Two years Physical Therapy experience required in at least 2 different levels of care, one being outpatient orthopedics. Bachelors Degree, Master's Degree of Physical Therapy. WI Physical Therapy license. Ability to communicate effectively verbally and in writing with staff, patients and families. Must possess a level of analytical skills necessary to determine need for physical assessments, to develop individual therapy treatment plans, to evaluate patient response to therapy and to modify treatment plan and interventions as appropriate. Must be very flexible and efficient.Specific Job Details:Hours are from 8:00 AM to 4:30 PM, may be flexible with these hoursMust be available to work 12 shifts in a month with 1-2 holidays as needed based on patient care and departmental needs.Description: Provides ongoing teaching/education to patients, family and health care team to optimize understanding and achievement of the patient's full potential. Evaluates/Re-evaluates patients to ensure appropriate treatment and discharge planning. Interprets evaluation results to establish appropriate functional, measurable goals. Designs and implements treatment plan to achieve goals. Works in all levels of care and facilities in Wheaton Franciscan Healthcare Rehabilitation Services. Must have skills consistent with treating patients in all levels of care.
