Program Supervisor

GENERAL DESCRIPTION:

The Supervisor for Supportive Housing is responsible for coordinating the day-today operations of his/her assigned Supportive Housing Services team.  The Supervisor fully participates as a member of an interdisciplinary team whose goals are to meet the basic needs, enhance the quality of life, and increase the social skills and independence of the consumers they serve. Each Supervisor is assigned to a specific team with clear and well defined program guidelines, funding requirements and evaluation components that will vary from other teams and for which they will be evaluated. The Supervisor is responsible to the Program Director.

 

SPECIFIC DUTIES:

1.               Coordinates the day-to-day operations of assigned team and is responsible for scheduling of meetings, services, staff time, and handling emergency situations.

2.               Coordinates the work of assigned team, includes providing direction, monitoring performance, and participating in staff performance evaluations, in collaboration with the Program Director.

3.               Responsible for quality assurance of assigned team’s service delivery, including clinical case assignment, documentation and transfer of cases to other units or agencies.

4.               Insures model fidelity, where indicated.

5.               Provides direct service; maintains reduced caseload.

6.               Monitors grant / contract compliance and compliance with accreditation standards. Prepares written reports as indicated.

7.               Fully participates in agency outcome evaluation system. Insures the collection of required data

8.               Identifies staff training needs and responsible for making recommendations to the Program Director.

9.               Monitors overall team efficiency and effectiveness and implements performance improvement activities when indicated. Recommends to Program Director changes that would improve service delivery and implements changes as indicated.

10.            Responsible for continuity of care in the provision of comprehensive services and coordinates communication with other service components of SWCS and outside agencies involved with SWCS clients.

11.            Works closely with the Program director to improve work flow processes.

12.            Assists in the development and implementation of new programming or initiatives as assigned by the Program Director.

13.            Coordinates training activities with the agency Staff Development and/or Clinical Consultants.

14.            Assumes responsibility for special projects and serves on agency work groups and initiatives as assigned by the Program Director.

15.            Assists Program Director in preparing quarterly and annual reports as required by contract/s.

16.            May be responsible for providing field instruction for students of his / her respective professional discipline as opportunities permit.

17.            Coordinates and participates in clinical case conferences.

18.            Adheres to the agency’s Policies and Procedures.

19.            Responsible for updating own knowledge in assigned area, keeping up to date with current trends in service delivery system.

20.            Maintains flexible work schedule, including evening and weekend on-call coverage.

21.            Adheres to SWCS' code of ethics and complies with the State of Michigan Mental Health Code

22.            Complies with SWCS standards for service delivery.

23.            Performs additional related duties as assigned by the Program Director.

KNOWLEDGE, SKILLS AND ABILITIES:

 

1.               Clinical expertise in working with individuals who have chronic mental illness and co-occurring mental illness and substance abuse disorders with histories of homelessness.

2.               Clinical expertise in crisis intervention.

3.               Knowledge and skills in assessment and person-centered planning.

4.               Knowledge of Assertive Community Treatment (ACT) model and other evidence based, promising and/or emerging practices as required by team assignment.

5.               Ability to provide leadership and direction to staff, support staff growth and create a positive learning environment

6.               Ability to work as part of a comprehensive community mental health team and to represent SWCS in the community

7.               Ability to engage homeless consumers in shelters and on the street and to engage consumers who are typical non-responders to traditional mental health/substance abuse treatment.

8.               Experience in program development and management.

9.               Working knowledge and ability to demonstrate leadership with the “Housing First” model.

10.            Familiarity with community support systems, health care and human service resources, court and police activities.

11.            Knowledge and belief of the recovery process for individuals with mental illness.

12.            Good organizational skills, administrative abilities, verbal and written communication skills and leadership capacity.

13.            Familiarity and working knowledge of computer applications, including word processing.

14.            Flexibility in days and hours available for scheduled work, including evenings and weekends.

15.            Knowledge of, and ability to, adhere to professional code of ethics.

16.            Knowledge of and ability to comply with Michigan Mental Health Code and / or other applicable standards.

MINIMUM QUALIFICATIONS:

 

1.               Possession of a Bachelors Degree in mental health discipline, Masters Degree preferred.  Appropriate professional registration / certification by state / national authority required (e.g., CSW, LLP, RN, LPC).

2.               At least three years professional experience in mental health / human services; managerial/supervisory experience preferred.

3.               Computer literate with working knowledge of word processing, preferably experience using Microsoft Office and Outlook e-mail systems.

4.               Possession of a valid driver's license and have access to a private vehicle for day-to-day job performance.
